Transition of Care

To assist you in finding a new healthcare provider, we have compiled a list of local primary care practices. It is always in your best interest to contact your insurer for recommended health providers covered by your specific insurance plan. We recommend reaching out to establish care as soon as possible and before our last day of clinical operations.

Medical Records

Pursuant to OAR 851-05500090(2)(a)(b), a copy of your medical records will be retained for 7 years by our records management company, Verisma. You can request a copy of your medical records by going here. If you have set up your patient portal to access your Hands On Medicine chart, this will remain active.

Outstanding Appointments

Scheduled appointments between now and October 18 are active. Hands On Medicine staff will be in contact regarding any appointments after October 18 and before the end of the 2024 year. All appointments after January 1 will be cancelled. We apologize for any inconvenience this may cause. If you find another provider or are unable to make a scheduled appointment before October 18, please call us and cancel, these appointment times are in high demand.

Outstanding Balances

If you have a balance with us, we need to be paid for all services already provided as soon as possible. Our accounts receivable department will remain active until all balances are managed. If you have any questions, please be in contact with the billing department.
